Applicant must possess a current active California license as Registered Dietitian, Bachelor of Science
degree, Current CPR certification, and Certified Diabetes Educator preferred (or willingness to pursue CDE).
Under the supervision of the Diabetes Program Coordinator and Director, the Diabetes Case Manager (DCM)
will conduct baseline assessments, develop treatment plans, and provide intensive case management and
education to patients with diabetes to achieve treatment goals. In addition, the DCM will provide appropriate
referrals and facilitate communication between Nursing, Medical, and Outreach. Must be able to work with
the Indian Community and be sensitive to the Indian culture and its needs.
