The Public Health Nurse serves as a member of the health care team and as an advisor to individuals and families, pregnant teens and young parents in home environment in the Indian Communities within an assigned geographic area under the supervision of the Community/Clinical Nursing Director. Goal is to reduce morbidity, reduce incidence of teen/unwed pregnancy, encourage good parenting skills, and health families, and appropriate immunization of specific age groups. This is considered a Category I position for potential exposure to Bloodborne Pathogens and other Hazardous Materials under OSHA.


Duties of this position shall include, but are not limited to the following:

  • Professionalism; ability to supervise and lead by example; committee participation; timely submission of reports.
  • Clinical performance; pre and postpartum home visits; chronic patient home visits.
  • Community development; health prevention/maintenance; health fairs/WIC.
  • Assist in providing prenatal and parenting classes.
  • Case management: patient care conferences with CHRs weekly.
  • Coordination and liaison activities: public health department, local hospitals, and medical clinics.
  • Immunization: coordinate with Site Manager, Childhood/flu vaccination.
  • Documentation: referrals, home visits.


  • Accomplish 80 home visits per month.
  • Does not compromise patient confidentiality.
  • Impacts on overall RSBCIHI services by active committee participation.
  • Consistently manages personnel effectively leading by example.
  • Conducts home visits on all identified postpartum clients within ten days of discharge.
  • Responds to referrals and properly documents within five working days of receipt. All reports are submitted in timely fashion.
  • Work with pregnant teens and young parents in home environment providing instruction on developmental stages of children, bonding techniques, and recognition of achievements.
  • Work with Licensed Family Therapist to develop family enhancement workshops to reduce the likelihood of teen pregnancy.
  • Initiate services at time of birth or as soon as possible.
  • Provide for intensive home visitations by staff for as long as necessary.
  • Provide for long-term service for families and children from 0-5 years of age (AIIHI).
  • Conduct risk assessments to identify families in need.
  • Arrange and coordinate caseload in order to maintain significant in-home contact with each family to meet their needs.
  • Assist in parenting and nurturing skills, self-esteem building.
  • Attend and participate in monthly staff and quarterly AIIHI work group meetings.
  • Coordinate and schedule patients for transportation as necessary.
  • Other duties as assigned.
  • This position requires working on weekends and evenings as needed.

Education & Experience


Bachelor of Science degree in Nursing from an accredited School of Nursing. Possess a Current California Nursing License and a Public Health Nurse Certificate. Healthy Families Training (within one year of employment)


Minimum of one year of Nursing work experience. Must be computer literate. Minimum of one year of supervisory experience preferred.


Specific abilities or expertise necessary for the position.


Must possess a valid California driver’s license. A current DMV printout is required

Current CPR Certification that meets the guidelines of the American Heart Association or Red Cross (Can be obtain prior to orientation)

Ability To

Maintain strict confidentiality, security and safety of health records.

Function as a member of the health care team by communicating clearly and effectively with patients, physicians and other staff members and the patient’s family.

Work a variable work schedule, evenings and weekends, to meet client and program needs.

Must be able to work with the Indian Community, and be sensitive to the Indian culture and its needs.

Physical Demands

The work involves periods of sitting, standing, and the routine risks of travel. Work is performed in the department office, in the clinic, in patients’ homes, in group settings, and outdoors, as indicated. Exposure to contagious and infectious diseases is a common risk.
